- Idling too long in Dark Woods while in a party may cause the game to stop responding. Relaunching the game will fix the issue. Xbox players must select "fresh session" to load into a new instance of Dark Woods.
- Players may not receive Daily Quests.
- Loading into a session with a party may encounter an error message or possibly reroute to lobby.
- Players in a party who have already permadied may revive when ascending to a boss fight.
- Players may be kicked from parties upon entering a session and may encounter a hang when returning to Dark Woods. Restarting the game will resolve this issue.
- The visual effects when spawning into Dark Woods is out of sync with when the Rebel Souls appear.
- Shield bubble from Sword of Justice co-op power may visually disappear if active while ascending.
- The icon for the Slow debuff may be missing on enemies when using the Torpor attack with the Staff of Sloth.
- Spamming actions after interacting with a gate delays a transition and allows for faster movement.
- Enemies may occasionally get stuck while following the player.
- Jumping Ragnos (the spider-like enemy in Inferno) may attempt to target players outside of their range, causing visual issues.
- Purified Vigile Timeweaver's (hourglass enemy in Purgatorio) AOE attack may continue after it is killed.
- Buffs received from the Soul Shrines may not be carried over to boss fights.
- Some visual effects are missing when you complete the Ascension Point battle.
- Lucifer's stalagmite attack can push players out of his arena.
- It's possible to escape Lucifer's arena is specific spots.
- Lucifer sound effects will play continuously if he dies during an attack.
- Adam and Eve will deal a small amount of damage during their introduction.
- Screenshake might seem too short or too long.
- Status effects may carry over between Training Sessions.
- Training Dummies may become permanently stunned.
- Effects from the Staff of Sloth will stay on Training Dummies when the player swaps weapons
- Some Cosmetic Feats may not function.
- Menus in Dark Woods can be interacted with while in the Pause menu.
- There is no visual indication when you don't have enough Shards in Charon's shop.
- Cost for locked Perk Slots will display incorrectly after viewing an equipped Perk.
- Attempting to open the Compendium from the Feats menu doesn't work.
- Players appear to have a shadow on them on the Run Results screen.
- The map legend hides the player icons on a small portion of the Inferno map.
- UI for the Chambers may show the incorrect number of players inside.
- Objective UI may be off-center when in Inferno or Purgatorio.
- Beatrice's dialogue can overlap the Training Select screen if the Training Flag is interacted with too quickly.
- Compendium entries may display incorrectly when viewed in a certain order.
- Placeholder text can be seen in the Feats menu.
- Typo when reviving all party members.
- One of the texts for a Rebel Soul C Feat has a typo.
- Sound effects for the Wrath of God sequences may continue to play if you quit before the sequence ends. Relaunching the game will fix this.
- Audio for Frozen Hate will continue if you quit during the attack. Relaunching the game will fix this.
- Music stops early when loading the tutorial.
- Music may stop when encountering large size mobs.
PC
- Performance issues may occur in Dark Woods as a result of memory leaks.
- Minimizing the game while loading boss fights might cause the bosses to load incorrectly.
- UI elements might be placed incorrectly when resizing the game.
- Display settings will default to the the wrong resolution when using the Restore button.
- Audio will still play if the Master Volume is set to 0 with a mouse.

- Some players may get stuck on a permanent loading screen when trying to enter Inferno.
- Players may crash when returning to Dark Woods
